COMMUNITY VOICES FOR 340B
RELEASE OF LIABILITY AND INDEMNIFICATION AGREEMENT

PLEASE READ CAREFULLY BEFORE SIGNING

I do hereby affirm and acknowledge that I understand the inherent hazards and risks associated with travel to, travel from, and participation in the above Events, including the risks of personal injury, heart attack, disability or death. Despite the potential hazards, I voluntarily agree to participate in the Events specified above and assume all such risks, known and unknown, and assume all responsibility for any injury, disability, death, or property damage. I believe myself physically fit and able to participate in the Events. I also understand that there will not be any medical assistance provided by CV340B during the Events. If at any time I feel at risk of injury, I will immediately cease further participation in the Events. I further understand that CV340B has no control over the transportation company that will be utilized to transport people to and from the March. I also understand that these events are not affiliated with the 340B Coalition Summer Conference.

In consideration of being permitted to utilize the transportation arranged by CV340B and in further consideration of the permission granted to me by CV340B and its officers, directors, employees, volunteers, representatives, and agents (hereinafter the “Released Parties”) to participate in the Events above, I hereby agree on behalf of myself and my personal representatives, heirs, next of kin, executors, administrators, and/or assigns, as follows:

1. To forever release, waive, indemnify, hold harmless and discharge the Released Parties from any and all claims, actions, causes of action, damages, demands or other liabilities, either in law or in equity, arising from or by reason of any bodily injury or personal injuries known or unknown, including death, and/or any property or equipment damage known or unknown which may occur as a result of on in connection with my travel to and participation in the Events.

2. By entering into this agreement, I am not relying on any oral or written representation or statements made by CV340B or any of its officers, directors, employees, volunteers, representatives, or agents, other than what is set forth in this agreement.

3. If any provision of this release is found to be unenforceable or invalid, that provision shall be severed from this contract. The remainder of this contract will then be construed as though the unenforceable provision had never been contained in this document.

4. I have read the participation rules and restrictions associated with the Events listed below and agree to abide by such rules and any instructions of Events organizers, Capital police, or other governmental authorities during the Events.

RULES/RESTRICTION OF PARTICIPATION

Rules for Demonstrating on Capitol, National Park Service, and D.C. Metropolitan Grounds include but are not limited to:

1. Always keep moving and allow all pedestrians to pass you.
2. Use only the approved CV340B signs given to you.
3. Keep noise to a respectful level.
4. Pick up after yourself and other Marchers.
5. Use common-sense safety measures.
6. Stay outside of all Capitol Buildings and off Capitol building steps.
7. Follow the pre-approved route.
8. Do not bring firearms, weapons, or any open flame on Capitol Grounds.

I understand that this is a legal document which is binding on me, my personal representatives, heirs, next of kin, executors, administrators, and/or assigns, and on any others who may claim by or through me. I am eighteen years of age or older, have full capacity to enter into this Agreement, and do so voluntarily.


I HAVE READ THIS AGREEMENT. I UNDERSTAND IT AND I AGREE TO BE BOUND BY IT.

I agree to allowing Community Voices for 340B to use my likeness through images and video in the form of but not limited to email, social media, website and print materials. If at any point I rescind my consent I will let CV340B know immediately and CV340B will take down and content including my likeness within 24 hours.
